The Open Swing Dance Championships is committed to providing a safe environment for all our attendees. We ask that all attendees review our Mission Statement and abide by our Code of Conduct.
The Open Safety Concerns Committee has implemented a Safety Violation Incident Form available online, for individual(s) who may need to report a personal safety concern or experience, or violation of The Open Code of Conduct.
The content of all reports submitted using this form will remain confidential and will initially only be reviewed by Heidi Dziak, Lead Safety Representative. Information reported may be shared with The Open Safety Concerns Committee with the permission of the reporter.

Heidi will respect your concerns for confidentiality and, if requested, will not share your personal identifying/contact information with anyone for any reason. Heidi will do her best to respond to report form submissions within one hour. In an emergency, call or text Heidi, or find a member of the committee.
